No emergencies, a light burden.
Nowadays, following Director Han’s orders, he didn’t need to join the hand-off meetings in the traumatic orthopedics department. He could rest and recuperate, waiting for instructions.
Yang Ping stayed at home, researching online all day, listing books on sports medicine and arthroscopy. He focused on studying sports medicine and arthroscopy during this period.
He could read in the system space, but reading required consuming points. By listing the required books in advance and finding them in the system space, he could save points.
In the afternoon, as soon as he had arranged all the material, Zhang Lin called him. A child of a pediatric colleague’s friend had a fracture at the femur condyles and wanted to see if he could help in setting the bone.
Yang Ping asked Zhang Lin to bring the patient to the orthopedic outpatient clinic and find a free consultation room, he would get there immediately.
The outpatient building was a separate structure, positioned to the front right of the Gemini Towers. Opposite it was the emergency building on the left. Between the two eight-story buildings was a small square, the hospital’s evacuation assembly point.
In the orthopedic outpatient clinic, most of the consultation rooms were fixed, but some backup rooms weren’t assigned to anyone in particular and were likely vacant.
Zhang Lin had a nurse open a consultation room, and he was already waiting there with the patient. Pediatrics, his girlfriend’s department, naturally he had to treat them well, so he personally escorted the patient.
The patient was a relation of Cai Junsheng, one of the infamous Sanbo studs, this was the first time Yang Ping had met him in person. He wasn’t a sloppy man, but rather handsome and trendy.
As soon as Yang Ping entered the consultation room, he shook hands with Cai Junsheng. Looking at the X-ray on the viewing screen, the fracture in the condyles of the right femur was misaligned and looked terrible.
“How did this happen?” Yang Ping asked the family.
The child’s father’s face flushed red and stammered, “This morning he fell off the horizontal bar, he landed on this side.” The father pointed to the elbow joint.
That didn’t sound right. Based on Yang Ping’s experience in manually setting bones, he had a clear understanding of the mechanism of injury and its results. He asked the family, “When the injury happened, did you apply any massage or pressure to the injured area?”
This type of fracture displacement typically happened as a result of a secondary injury.
When a child is injured, the biggest mistake parents make during initial treatment: first, they spray some topical medicine, then give it a vigorous massage, with the intention of dissipating the blood from the bruise.
This kind of treatment would take a minor injury and make it severe.
Patients often hide their medical history for various reasons, so it was crucial to ask clearly, only then would it be better for the treatment.
“No, we came straight here after getting an X-ray,” the child’s father said.
The child’s mother seemed unhappy, as if she had something to say, but remained quiet.
The child spoke up, “But my mum and dad helped me pull it for a long time, it hurt so much.”
Yang Ping, Zhang Lin, and Cai Junsheng were all shocked. Zhang Lin said, “Quick, tell us what happened. Explain it clearly, otherwise Doctor Yang will have to operate soon, and there won’t be time to set the bone.”
The mother, unable to hold back, spilled the whole story.
It turned out that the father was an avid DIY enthusiast. When the child fell, he took him to the hospital.
After an X-ray, the doctor said there was a fracture and displacement. The doctor suggested setting it manually and stabilizing it with percutaneous pins, a minimally invasive surgery. This meant anesthetized adjustment in the operating theatre, the skin would be pierced by a few slim steel pins to stabilize the fracture, which would be removed four weeks later.
Just from a little fall, and now needing surgery, and inserting pins, the father thought the doctor was exaggerating the situation.
He immediately made a wise decision, they would go home first and see.
After searching around online and watching videos, he felt the treatment was very simple, just pull a little, then flex the elbow joint. It was so simple, even easier than fixing a television. He decided to set the bone himself. He could use hard cardboard for fixation and buy a brace for the elbow joint online, waiting for it to be delivered. All this cost less than a hundred.
But he couldn’t pull it by himself. According to the video, at least two people were necessary. So he asked the mother for help but she was reluctant.
The father reassured her, “It’s simple, I’ve learned how to do it, don’t worry, just pull a little,” then he threatened her a little, “If we go to the hospital, it would definitely require surgery.”
The mother, with her soft heart, agreed to help, and they pulled so hard the child cried.
After pulling, they used cardboard for fixation, then confidently felt the spot and announced, “It’s done.”
The mother was unsure and insisted on going to the hospital to get another X-ray. When they got to the hospital, they headed straight to Emergency. The father told the doctor, “Doctor, I want an X-ray, the right elbow joint, just an X-ray, nothing else.”
The doctor was puzzled, “Are you also a doctor?”
The child’s father just grunted.
When the X-ray came out, it was shocking—the fracture displacement had increased several times. Even a layman could tell that it was a joint before, but now it was completely disassembled, in a complete mess.
The child’s father’s hands were shaking—he’d messed up!
At home, he managed all the repairs himself when the refrigerator or TV broke after the warranty period–there were even times when all the parts were scattered, and he could call the manufacturer to clean up the mess.
There was no manufacturer to call this time.
The mother looked at her husband’s pale face, she knew something had gone wrong. She compared the two X-rays to two reports and said, “The displacement is significantly worse than before.”
“You’ve broken it. Were you playing around with our son?” After scolding him profusely and causing a scene with tears, punches, and kicks, the child’s father took all the blame and continued to call his friends for help. After many turns and turns, he found Cai Junsheng.
There were instances of people doing much more dangerous things than this at home, like watching surgical videos online and circumcising themselves, then rushing to the emergency room with blood-soaked pants because of the wounds.
Ignorance is truly bliss!
“You think flying a plane is as simple as pressing a few buttons?” Zhang Lin said bluntly.
The child’s mother pleaded anxiously, “Doctor, please find a solution.”
“We need to reposition it under anesthesia, use percutaneous pinning instead of surgery. This type of fracture cannot be fixed with a cast, we have to use pins. The ends of the pins will be left outside and removed in about four weeks.” Yang Ping explained.
A standard procedure for common fractures, though the father had made the repositioning much more difficult, but this wasn’t a problem for Yang Ping.
“Great! Doctor, I’ll sign the paperwork. Please arrange the surgery as soon as possible.” The child’s mother said decisively.
“Do you want to perform it in the outpatient operating room? But we don’t have a C-arm X-ray machine there.” Zhang Lin said.
Yang Ping assured him, “We don’t need a C-arm X-ray machine. A drill and Kirschner wires will do.”
After so much practice in repositioning, he could tell what was needed by touch.
They signed the form to prepare for surgery.
The outpatient building had its own operating room, usually for minor surgeries like ganglion cysts for orthopedics, or abortions for gynecology.
The surgery itself was simple. The anesthesiologist did nerve blocking, then disinfected and laid out the sheets. The standard procedure was followed to reposition the bones, using two wires on the radial side and one on the ulnar side, and the three Kirschner wires were crossed for fixation.
All the while, the child’s dad was being constantly scolded by his wife. His head hung low, unable to lift it up from the shame. His treasured image of a high IQ technical man, built up over the years of DIY, had crumbled to the ground.
“You big idiot–” The child’s mom concluded with a vicious scolding.
The child’s father didn’t dare to retort even a word. Sweat was dripping from his forehead as he let his head hang low.
After around ten minutes, the surgery was completed. The kid was pushed out and sent to radiology for X-ray. The fracture was perfectly repositioned and the Kirschner wires were in good shape.
The parents were instructed to let the child rest in the outpatient department for around fifteen minutes and could then go home. They were advised to return for a check-up after a week, and to not move it around at home excessively. If there were any issues, they should promptly return to the hospital.
After they were given the instructions, the child’s parents expressed their immense gratitude. Cai Junsheng also showed his gratitude and Zhang Lin was pleased.
Cai Junsheng thanked Zhang Lin.
Zhang Lin replied, “No need to be polite. We’re all on the same side.”
Yang Ping used his free afternoon time to borrow a manual for the arthroscope from the equipment department and then went to the operating room to study the arthroscope.
There was a spare one in the storage room.
He studied the function of each button and each connection on the arthroscope carefully, using the manual as a reference.
He could accurately recall any detail of the equipment even with his eyes closed.
