Boss Cheng yearned for the surgery to be conducted, yet when Doctor Yang confirmed it could be done, he felt a twinge of hesitation.
Can it truly be done?

“Prepare the patient for surgery!” Su Nanchen repeated what Doctor Yang had said.
Now that he had Su Nanchen’s affirmation, Boss Cheng felt relieved, “Prepare the patient!”
The ward doctor immediately contacted the operating room. The surgery, as planned, was ready to proceed and the patient was to be prepared immediately.

The patient had already been explained the probable situations, so the preoperative preparations were well in hand and they could enter the operating theater at any moment.
Once the patient was transferred to the operating room, Boss Cheng accompanied the two to his office for a brief rest. Doctor Yang refrained from discussing the upcoming surgery and just sat back, relaxed.
Boss Cheng wanted to ask, “Doctor Yang, how many such surgeries have you conducted?”
But he figured such a question would imply mistrust. To distrust Doctor Yang would mean to distrust Su Nanchen. Therefore, he swallowed the words that were at the tip of his tongue.
Seeing Boss Cheng’s apprehension, Su Nanchen sensed what was troubling him.
“Isn’t the reconstruction of the femoral condylar fossa a bit tricky?” Su Nanchen broached the subject.
Doctor Yang responded nonchalantly, “Ah, it’s not a big issue, just do some shaping with a burr.”
It was easier said than done. Everyone knew shaping with a burr was necessary; the challenge was how to do a personalized shaping, while also fulfilling all the surgical requirements.
Upon seeing that Doctor Yang wasn’t at all worried, to the point where he didn’t even see a need to discuss it, Su Nanchen didn’t ask any further. The recurrent image of the video that showed the five-structure reconstruction surgery became his source of confidence in Doctor Yang.
After they had rested enough, Boss Cheng led them to the operating room.
Excited to see Doctor Yang and his party, Jiahui enthusiastically greeted them: “Hello, Professor Yang! Hi, Professor Su! Hello, Boss Cheng!”
Doctor Yang gave a slight nod acknowledging her greeting.
Jiahui felt delighted. Doctor Yang was so prestigious, even being invited here to perform surgery. It was evident that both Professor Su and Boss Cheng held him in high regard.
The patient had already been anesthetized. Two doctors were busy attending to him: positioning the patient’s body, applying the drape, disinfecting their hands, covering the area with sterile sheets. Two doctors from this hospital were assisting on the stage, with others observing from below.
Intrigued, everyone wanted to see how he would execute the surgery, especially the shaping of the femoral condylar fossa, which seemed virtually impossible. “Anterior cruciate ligament reconstruction, tibial locator, pilot-hole drill!”
The nurse was still not able to keep up with the pace and was not used to this rapid speed. To keep the surgery compact, Doctor Yang had to notify the nurse in advance to prepare for the next step.
“Next, prepare the tibial tunnel drill!”
The moment the locator was released, the positioning was accurately done in a single attempt, and two tunnel paths were quickly established by the pilot-hole drill.
“Dr. Su, what is the length and diameter of the anterior cruciate ligament allograft tendon?”
Remained busy in processing the tendon, Sun Nanchen promptly reported the measurements.
“Tibial tunnel drill!” Leaving two pilot-hole drills, Doctor Yang took off the electric drill and handed it to the nurse.
There was a slight disconnect in the coordination. He had to wait again. If it was Xiao Su assisting him, he wouldn’t have needed to instruct. Each step, every piece of equipment, would be handed over at the right moment, ensuring a smooth flow of the procedure.
After a while, he received the tibial tunnel drill, and two tibial tunnels were established at the precise locations.
“Femoral locator, prepare the pilot-hole drill!” Doctor Yang removed the two pilot-hole drills from the tibial tunnels.
The nurse handed over the femoral-end locator. Before her hand could even retreat, Doctor Yang had already positioned it correctly, pointing directly at the geometric center of the ligament femoral footprint.
After waiting for a while, he received the electric drill. The needle securely held the center of the ligament footprint. Surprisingly, even without any estimation or measurement, the needle tip was accurately positioned. Sandy sounds were emitted as the needle drilled through, and the exit point was ideally positioned.
“Femoral tunnel drill!”

“Pass the guidewire needle!”

“It’s okay! Take your time, prepare the tools for the posterior cruciate (PCL) reconstruction!” Doctor Yang reached to the instrument table to grab the guide needle himself.
Boss Cheng immediately ordered, for another surgical nurse to be brought in, as it was clear one wasn’t enough. They were all tensed, judging from their furrowed brows.
The tendon’s thread was pulled through, given a tug, and then pulled back. Before anyone could get a clear sight, the tendon was already implanted, caught by the toggle plate at the femur end.
“The compression screw!”
Another nurse hurriedly went to scrub up. The surgical nurse on the table took a deep breath, as she couldn’t catch up. Boss Cheng couldn’t keep his calm. Although troublesome, he had to admit that it wasn’t because the nurses were incompetent, the surgery was just too fast. He ridiculed himself thinking that his past surgeries seemed like chasing bugs in comparison.
The compression screw was passed over, rotated in, and the excess allograft tendon was cut off. The moment its tools were pulled back, Yang was already waiting for the guidewire for the posterior cruciate ligament (PCL).
Yang didn’t express any annoyance or speak. He quietly waited. When another nurse ascended to the stage, he personally adjusted the patient’s position on the operating table to suit the next phase of the surgery. As he initiated the double bundle reconstruction of the PCL, the assistants who were holding the leg were left dizzy by the swift exchange of various instruments and adjusting of angles. They couldn’t keep up with the pace neither mentally nor physically.
Another nurse stepped up to the platform with their surgical gown on. One prepared the instruments currently in use, and the other prepared the instruments for the next step.
The guiding needle, swiftly, one for the tibia, one for the femur. Both tunnels were opened again.
The drill bit for the femur and tibia was needed again. Finally, when the drill bit was assembled properly, the rhythm improved with two surgical tables now, despite being a bit chaotic.
Su Nanchen was engrossed and dedicated himself to help with the organization of the PCL’s tendons, while occasionally glancing at the monitor displaying the surgery.
The tunnels made were perfect again. Su Nanchen finally managed to keep up this time. The tendon was barely handed over and it was immediately time to optimize the tendon for the reconstruction of the posterolateral and posteromedial structures. He couldn’t even afford to take a breather, let alone make casual comments during the surgery.
Boss Cheng coordinated from the side, commanding nurses and doctors to keep up the pace.
The reconstruction of the PCL was done in a flash, the posterolateral, posteromedial, each three structures, were done with swift, precise incisions. Small cuts were made, tunnels were created, and tendons were implanted!
One step done, then a pause. All eight structures, both the anterior and posterior cruciate ligaments with double bundles, ten tendons were all implanted.
“Repair the joint capsule!”
The doctor holding the leg was shaking. Seeing the needle and thread passed over, he only observed the needle going in and out— in a daze, the joint capsule was fixed.
“Alcohol gauze!”
It was time for the disinfection of the skin with 75% alcohol before suturing the skin.

However, today’s surgery was done so efficiently that he felt he would embarrass himself if he were to step up to the plate. One hour had barely passed, and already the four-in-one reconstruction was complete.
Su Nanchen also breathed a sigh of relief. It was only by performing at full speed that he barely managed to keep up.
The few observing doctors had their eyes opened to a new spectacle. They had initially wondered why Su Nanchen had invited him, but now they saw how formidable this guy’s surgical skills were.
As Doctor Yang stepped away from the operating table, preparing to take off his surgical gown, Jiahui, who was already standing at his back, promptly helped him untie the strap.
“I’m going to wash my hands!” Dr. Yang, despite having already removed his gloves, said out of habit.
Jiahui promptly led the way to the handwashing area.
Su Nanchen hadn’t managed to change out of his surgery gown yet, and Boss Cheng was standing there, unable to react for a moment.
Su Nanchen stripped down to check the stability of the knee joint again. He checked it several times, forward and backward, internal and external rotation, it was very stable. There wasn’t much difference compared to a normal knee, and the wobbly preoperative knee was nowhere to be seen.

This was his first time watching Doctor Yang’s live surgery and it was even more shocking than watching a video.
